Revs up and down at an idle and dies when I put it in gear

On my 92 s10 4.3, it revs up and down at an idle
then dies whenever I put it into gear. I have to keep
it revved up with one foot and apply the brake with
the other for the first few minutes of driving it. It
will run rough for a while then clear up, but as soon
as I kill it, it's the same thing all over again. This
started off as an every now and again thing and
slowly gotten worse.  Everything to do with fire is
new, new Throttle position sensor, repaired
damaged vaccuum lines and check valve, new fuel
filter. Any ideas???
